Rumination: Evidence-Based Coping Strategies
RFCBT, Mindfulness, and Metacognitive Approaches
What is Rumination?
Rumination is the repetitive, passive focus on symptoms of distress and their causes and consequences, without engaging in active problem-solving. It is a transdiagnostic process found in depression, anxiety, OCD, and PTSD.
How Rumination Differs from Problem-Solving
- •Rumination: “Why does this always happen to me?” (repetitive, abstract, past-focused)
- •Problem-solving: “What can I do about this?” (concrete, action-oriented, future-focused)
Evidence-Based Strategies
Rumination-Focused CBT (RFCBT)
- •Developed by Edward Watkins at University of Exeter
- •Trains individuals to recognize rumination triggers
- •Shifts from abstract/evaluative thinking to concrete/experiential thinking
- •Uses functional analysis to understand rumination’s costs
Behavioral Activation
- •Scheduling meaningful activities to break rumination cycles
- •Reducing isolation and withdrawal
- •Engaging in absorbing, flow-inducing tasks
Mindfulness-Based Cognitive Therapy (MBCT)
- •8-week program combining mindfulness and CBT
- •Teaches observation of thoughts without engagement
- •Particularly effective for preventing depressive relapse
- •Body scan, sitting meditation, and mindful movement
Metacognitive Therapy (MCT)
- •Targets metacognitive beliefs about rumination (“worrying keeps me safe”)
- •Detached mindfulness technique
- •Challenges the belief that rumination is useful
Quick Techniques to Interrupt Rumination
- Attention shifting — Focus on sensory details (5 things you see, 4 you hear, etc.)
- Scheduled worry time — Limit rumination to a specific 15-minute window
- Physical movement — Exercise disrupts ruminative thought loops
- Externalizing — Write thoughts down to create distance
- Self-compassion — Treat yourself as you would a friend
